How to complete my postal
kit
You should not wee for 2 hours before
to doing this test!
- Fill out the yellow sections
of the form inside the postal pack. We must have the
following information to process your test. We need
your name, your d.o.b, your Huddersfield/ Calderdale
full postcode, and a means of contact i.e mobile number,
land line number, or full postal address.
- Complete the sticky label
on the left hand side of the form and attach it to
the sample bottle. Make sure your name and d.o.b are
the same on the label as they appear on the test request
form.Pass the first part of your urine into the bottle
(about half fill the bottle).Put the lid on once you've
done the sample and tighten the bottle top.
- Put the sample bottle containing
the urine into the cardboard container, and secure
the cardboard container lid tightly.
- Put the completed form and
cardboard container containing the urine sample inside
the plastic envelope on the back of the sample request
form, and seal the bag.
- Put the form and sample into
the green/ white stripped envelope and post it in any
Royal Mail post box. You are NOT required to pay postage.
